Fees & Insurance

Payment

Services are provided on a self-pay basis at this time

Visit Fees

Initial psychiatric evaluation (60 minutes): $250

Extended Follow-up Visit (more than 30 minutes up to an hour): $200

Standard Follow-up visits (less than 30 minutes): $150

Administrative Services

Forms, letters, or paperwork completed outside of a scheduled visit may be billed at $75 per request.

Cancellation Policy

Appointments cancelled with less than 24 hours’ notice, or missed appointments, may be subject to a $75 fee. This helps ensure appointment availability for all patients. This fee must be paid before a new appointment is scheduled.

Insurance

I am not currently contracted with insurance plans, but this is something that I am working on. You may be able to use out-of-network benefits depending on your plan.

Out-of-Network Reimbursement

A superbill can be provided for you to submit to your insurance company for possible reimbursement.

Payment Methods

Payment is due at the time of service. Accepted forms of payment include credit and debit cards.

About Me

My name is Cindy Dudley. I am a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) with dual certification as a Family Nurse Practitioner (FNP). I provide evaluation, diagnosis, and medication management for adults.

My background includes working with a wide range of individuals, including those with intellectual and developmental disabilities and serious mental illness, in both inpatient and outpatient settings. This experience has helped me recognize the importance of providing individualized care based on each person’s specific needs.

I work with adults experiencing a range of mental health conditions, including anxiety, depression, mood disorders, PTSD, insomnia, and ADHD.

I focus on practical, manageable treatment plans and ongoing collaboration, with the goal of helping you feel better and function more consistently in your day-to-day life.
